Overview
Ohio Soccer Association (OSA), in partnership with Players Health, provides general liability, participant accident medical, and Directors & Officers insurance coverage for their full member leagues and clubs while conducting operations on behalf of OSA.
Players Health is a mission-based athlete safety and insurance company focused on creating the safest environment in youth sports. As founder of the company, Tyrre Burks had a vision to create a company that tackled athlete safety issues such as abuse prevention and injury management in sports. OSA chose to partner with Players Health because of their expertise and willingness to provide creative solutions to meet the needs of its members.
The insurance information outlined is for general information to what coverage(s) the insurance policy or policies provide and is not intended to describe all of the various details pertaining to the insurance. Actual coverages are detailed in the policy of insurance and are always subject to terms, provisions, conditions, and exclusions as contained therein.
Coverages
General Liability
General Liability insurance protects the Ohio Soccer Association and its members. The insurance covers everyone involved with OSA and clubs, like players, coaches, and volunteers. It covers injuries to people and damage to property that happen during soccer activities approved by the association. It also covers injuries on outdoor fields owned by other groups when the association or members use them. The insurance also helps if someone accuses the association of doing something wrong, like false arrest, libel (written defamation of character), or slander (spoken defamation of character). It even covers accidents with cars the association uses for its work, but not if they’re carrying players. Lastly, it protects the association if people get sick from food or drinks sold at their events. This insurance also covers sexual abuse and molestation coverage for organization and members.
Participant Accident Medical
Participant Accident Medical insurance helps when players, coaches, and other team members get hurt while playing soccer. It covers injuries that happen during games, practices, tryouts, and special events supervised by team officials. It also covers injuries when the team travels together for a game or event. This insurance is provided by Great American Insurance Company and is a backup if the person already has other health insurance. But if they don’t follow the rules of their primary health insurance, the coverage may only pay half of the medical expenses.
Directors & Officers (D&O)
Directors & Officers insurance is for the Ohio Soccer Association and its full members, like leagues and clubs. It covers the people who have important roles in the association, like directors, officers, and volunteers. The insurance protects them if someone blames them for doing something wrong in their roles. It covers legal costs and settlements up to $1,000,000 for things like lawsuits and claims related to employment practices. The insurance has deductibles, which are the amounts the association has to pay before the insurance helps. However, it doesn’t cover injuries, property damage, certain lawsuits, or things like sexual abuse.

Players Health supports Ohio Soccer Association members by providing a simplified way to access various insurance coverages and related benefits, both personally and professionally. These include:

- Club Benefits and Insurance: Offering travel insurance, workers’ compensation, crime insurance, equipment protection, event cancellation insurance, and payroll services for sports clubs.

Participant Accident Medical Claim Procedures
In the event of injury requiring medical treatment:
1. You as a member of Ohio Soccer (Injured individual, guardian, or parent) file a claim on PH Claims site.
2. An incident/FNOL (First Notice of Loss) is created by the PH system and included on Ohio Soccer dashboard.
3. The incident/FNOL is verified by the Ohio State Soccer Administrator and either approved or denied.
4a. If denied by Ohio Soccer, will explain the reason why and it will be sent in an email to you.
4b. Incident/FNOL becomes a claim and moves into Claims dashboard.
5. Co-ordinated Benefit Plans (CBP), the Third-Party Claims Administrator, pays claims on behalf of Great American Insurance. CBP will send an email acknowledgment with further instructions to the claimant once they received the approved FNOL. You will need to provide CBP with itemized medical provider bills (i.e., CMS-1500 / UB-04) for all services rendered as well as Explanation of Benefits (EOB) from your primary health insurance plan. It is the claimant’s responsibility to request these items from the appropriate parties.
6. Claim is paid based on the insurance policy benefits.
Certificate of Insurance
A certificate of insurance is an official document that provides proof of an insurance policy held by a sports organization. It contains essential information about the insurance coverage, such as the insurance company’s details, policy type, coverage limits, and effective dates. This certificate is necessary for the organization to demonstrate its financial preparedness in case of accidents or injuries during sports events. It also offers peace of mind to other involved parties, assuring them of protection from potential risks. Ultimately, the certificate of insurance allows the sports organization to focus on providing a safe and enjoyable sports experience without unnecessary worries about financial liabilities.
Adding an additional insured to a sports organization means including another person or entity on the organization’s insurance policy, extending coverage and protection to them as well. This is often required by venues, event organizers, or other collaborators to ensure they are safeguarded during sports events or activities. For sports organization administrators, adding an additional insured is crucial because it demonstrates a willingness to share insurance protection and provides reassurance to other parties involved. By doing so, the organization ensures that everyone participating in the events is adequately covered and protected from potential legal claims or financial losses. However, it’s essential for administrators to carefully review the terms and conditions of adding an additional insured to understand any specific requirements or limitations that may apply to the coverage.
A Certificate of Insurance (COI) is valid for one seasonal year, September 1 – August 31.
COI’s are emailed at the end of August/September 1 each seasonal year directly from Players Health to current OSA members. OSA Annual Memberships must be renewed and paid before COI’s are issued.

To request a COI that expires August 31st each seasonal year please click the “Request COI” button below.
To request a COI:
- Enter the facility name and full address. For a school, the Board of Education or Administration Office address is normally used to cover all facilities. Please verify the proper address with your school contact before submitting your COI request. If you need more than one league listed on your COI, type all league names under “Event Name”.
- Select your league name from the dropdown box. If you participate in multiple leagues, please include the additional league name(s) under “Event Name”. Only league name(s) will be listed on the COI, however all team(s) affiliated with the league(s) will be covered.
- Fill in your Club/Team contact information, add any special wording the facility requires, and click the “Submit” button. Please note that adding special wording may delay processing.
You will receive the COI via email directly from Players Health Certificates at phcoi@playershealthcoi.com. Please allow two (2) business days for processing.
