Complete the online annual Member Application
In order to complete, you must be listed as a user in your club/league GotSport system.
GotSport Dashboard > Affiliations > OSA > Available Forms > OSA Club & League Membership Form
Remit your $50 annual membership fee
Add and/or update your officers and contacts under "Users" in your GotSport system to ensure D&O coverage
Upload a current Governance Document into your GotSport system to update our new process/records
GotSport Dashboard > Affiliations > OSA > Available Forms > OSA Member Governance Doc
