Overview
Registration to Ohio Soccer Association is a vital and necessary part of the membership process. We pride ourselves on knowing who our members are in order to communicate effectively and cater to your needs, as well as provide a host of other benefits for your organization.
OSA utilizes GotSport as our registration and Safe Soccer (risk) training platform which satisfies all state and federal Safe Soccer requirements. All members will utilize the GotSport system in some fashion. The requirements can vary by program type.

Annual Membership Form
Membership in Ohio Soccer Association consists of both leagues and clubs, and is renewed on an annual basis. Members gain voting rights for board elections, among other things.

Forms
OSA now processes its player permissions through GotSport. It is recommended to select forgot password first, to see if you already have an existing GotSport account. If you do not have an account, follow the instructions to create an account to proceed.
Permission to Travel: Teams no longer need to obtain permission to travel prior to attending an event/tournament outside of OSA. Once a competitive team has been approved by their OSA member league, the bottom of your roster will state “OSA Official Roster is valid for all OSA sanctioned activities including permission to travel”.
PLAYER PERMISSION FORMS
OSA Carded Tournament Guest Player: This is ONLY for an OSA carded team requesting an OSA carded player to guest play with their team in a tournament. To be completed ONLY by the Team Coach/Club Admin using their personal GotSport account.
Out of State Tournament Guest Player Permission: This is ONLY for an OSA carded player wishing to guest play with a team from another state. To be completed ONLY by the parent/guardian using their personal GotSport account and selecting their player’s name.
Out of State Player Permission
[Ohio Resident] For players who live in Ohio but wish to play on a team in a surrounding state, please complete the Ohio Out of State Player Permission form.
[Non-Ohio Resident] For players who play with an OSA member club but reside in a different state, those players must receive permission from the state association where they live to play in Ohio. Must be completed annually. For example, if a player resides in Indiana but plays for a club in Ohio, the player is to receive permission from Indiana.

Your request will be processed within 2-business days and automatically notify the accepting State Association once approved, except for requests to play in PA-West, which will still require OSA to notify the State Association directly.
Tournament Only Team Registration
Tournament Guest Player Pass Registration: This is ONLY for a player registering as a Tournament Guest Player who is NOT currently carded to any OSA team. To be completed ONLY by the parent/guardian using their personal GotSport account and selecting their player’s name.
Non-GotSport Users: For clubs/teams who are not currently using GotSport and are not OSA members.
Current GotSport Users: For clubs/teams who are currently have a GotSport club account or are OSA members.
- Create the team(s) in your club system and roster all players and coaches.
- Register the team(s) to the 24/25 Tournament Teams Registration.
NOTE: An OSA risk approved coach must be on the team in order for you to register. - Notify Jessica Eberle [jeberle@ohio-soccer.org] that you have registered your team(s).
- An invoice for $21/player will be generated for all rostered players.
- Rosters and cards can be printed ONLY after all players have been approved, coaches have completed their OSA risk and payment has been received.

Grassroots/Recreational Registration
Our goal is to make registration to Ohio Soccer Association as simple as possible, while assisting you in complying with the necessary state and federal requirements. All player and coach registrations are handled via our registration partner, GotSport, through two (2) distinct user options.
- Using GotSport for your registration and directly registering to OSA
- Uploading data into GotSport and directly registering to OSA
Grassroots coaches and adult participants are responsible for completing their Safe Soccer (risk) training in GotSport. Their OSA registration is not complete until they satisfy OSA’s risk requirements each seasonal year. In the event of an incident or claim, they may not be covered until properly registered and risk approved.

- Player Registration Benefits: OSA automatically includes the three (3) required annual OSA forms (OSA Player Registration Form, Lindsay’s Law, and Concussion Information Sheet) directly in GotSport.
- Coach Registration Benefits*: Offers a more seamless user experience in completing/renewing OSA risk requirements each seasonal year.
*If you are collecting coach registrations in GotSport, we recommend creating a separate registration program for your coaches/volunteers.
Direct Register To OSA
Please ensure all players and coaches have an affiliation = USYS and a competitive level = Grassroots before directly registering them to OSA.
- From your league GotSport Dashboard, click on “Players” (can also be found under Club Management on the left-hand side)
- Bulk select/filter your USYS/Grassroots players (make sure to select all players in collection)
- Click on the blue “Register” button
- Send Selected Players To = Ohio Soccer Association
- Seasonal Year = 24/25
- Level = Grassroots
- Click on the blue “Register” button
- Repeat process for your coaches/volunteers
Recreational leagues who are not collecting registrations in GotSport will need to upload their player/coach data to OSA into GotSport.
- Use the following templates to import your player/coach data into GotSport:
- Player Upload Template (Competitive Level = Grassroots)
- Coach Upload Template (Competitive Level = Grassroots)
- Save your templates as .csv files
- Email your player/coach template files to members@ohio-soccer.org OR upload your player/coach templates directly into GotSport
- To Upload Into GotSport:
- From your league GotSport Dashboard, click on “Players” (can also be found under Club Management on the left-hand side)
- Click on “Upload Players” at the top
- Select Affiliation = USYS
- Choose File
- Open Selected File
- Confirm all columns are correctly mapped
- Import Data
- You will receive an email once your upload processes
- Click on the link in the confirmation email to reconfirm if your data successfully imported
- To Direct Register To OSA:
- From your league GotSport Dashboard, click on “Players” (can also be found under Club Management on the left-hand side)
- Bulk select/filter your USYS/Grassroots players (make sure to select all players in collection)
- Click on the blue “Register” button
- Send Selected Players To = Ohio Soccer Association
- Seasonal Year = 24/25
- Level = Grassroots
- Click on the blue “Register” button
- Repeat process for your coaches/volunteers
- OSA Safe Soccer Certification Form: Complete to certify your recreational league has collected the three (3) required annual OSA forms for the season.
- From your league GotSport Dashboard, click on “Affiliations” at the top
- Under OSA, select the blue “Available Forms” button
- Click on the 24/25 OSA Grassroots Safe Soccer Certification Form white “Submit” button
- Log into your Personal GotSport Account as the League Admin/Official and select your name to complete
OSA Required Annual Forms
OSA Invoices
Once you have directly registered your players and coaches, invoices will be generated on a monthly basis and can be accessed from your league GotSport Dashboard under “Registration Billing”. You will only be invoiced for the players you directly register to OSA. The first seasonal year invoice will be generated in October. The final seasonal year invoice will be generated in July.
- 1st/Month: Invoices generated
- 30th/Month: Invoices due
It is VERY important that you submit you player/coach data to OSA to establish membership and insurance coverage in the event on an incident or claim each season.

Competitive Registration & Carding
Player registration for carding to a competitive team occurs via the club at which the player wishes to participate. Cards are issued by the competitive league a team participates in.
All clubs participating in OSA member competitive leagues card through the GotSport system. If you are a new club that needs to be set up, please contact our Member Services team: members@ohio-soccer.org or 330-667-0850.
Please ensure all players and coaches have an affiliation = USYS and a competitive level = Competitive to trigger the appropriate OSA player and coach requirements.
Competitive players will need the following completed in GotSport in order to be approved for carding:
- Headshot photo
- Proof of birth
- OSA Player Registration Form*
- OSA Lindsay’s Law*
- OSA Concussion Information Sheet*
- Out Of State Permission for Non-Ohio Residents
*These three (3) OSA waivers must be completed by a parent/guardian directly in GotSport.
Competitive coaches and adult participants are responsible for completing their Safe Soccer (risk) training in GotSport in order to be approved for carding.

Age Group Chart
When determining the age group for a season, the year the season ends should be used for determining the birth year. Also note that the format “Age followed by and under” really means that age and younger. For example, 2015 birth year in the 2024-25 seasonal year would be listed as under-10s.
